Job Summary
Coordinate the patient's care to increase the efficiency and quality of care that is delivered to the patient. This role will start with the time of the patient's diagnosis and will continue through the patient's journey throughout their care. Will be the patient advocate and care for them in a highly supportive and personal manner and also will serve as a liaison to ensure that patients and families receive resources and services as needed.

Essential Functions

 

  • Ensure continuity of care with discharge appointments for all observation patients following catheterization
  • Check on any admitted patients following catheterization to be sure any ancillary testing (EKG, ECHO) has been completed for discharge and that they are ready to be discharged
  • Assist with ensuring appropriate records are obtained and available for review
  • Assist with coordination of catheterization with other procedures (communication with other departments/additional scheduling, etc.)
  • Maintain working knowledge of catheterization schedule and assist in troubleshooting problems
  • Being a primary point of contact for cardiologists/physicians who are referring patients to LCH for cardiac interventions and provide them updates of their patients throughout the process.
  • Coordinating the plan of care for new patients and maintain a relationship with them and their families throughout the course of their care, from meeting them at the initial consult, then being present during their hospitalization and available via phone directly post hospital discharge.
  • Calling patients/families to answer questions they have about our program, setting them up for initial consult
  • Schedule radiology scans for patients of outside cardiologists who prefer that their patient’s CT/MRIs be performed at Atrium
  • Calling the patient’s parent(s) the day prior to their procedure to confirm there have been no recent illnesses prior to coming
  • Triage all cath related phone calls and e-mails of patients/parents who have concerns post discharge (incision infections, medication questions, pain management, etc).
  • Fill out all FMLA paperwork for families as requested and write all school excuse and activity restriction letters.

Physical Requirements
Ability to lift objects weighing up to 25 pounds. Occasional stooping, kneeling, crouching. May require sitting/standing for long periods of time.

Education, Experience and Certifications
BSN Degree Required/MSN Preferred. Current licensure to practice as a Registered Nurse in North Carolina required. Certification in the appropriate specialized field within 2 years of being in the position required. Excellent communication skills required.
